Possible sources of pain

The calf muscles help push the foot down and absorb force during walking, running, and jumping. A muscle strain may cause a sudden pull, tightness, or pain when you contract or stretch the calf. A muscle contusion can follow a direct blow and may bring tenderness or bruising. Achilles tendinitis can cause pain lower down near the heel, sometimes with stiffness during the first steps. Hands-on treatment, including sports massage, should wait until a clinician has considered a significant tear or other cause.

Arrange assessment if bruising or swelling is extensive, pain prevents normal walking, or symptoms are not steadily improving. Seek urgent care for sudden calf swelling with warmth or redness, severe tenderness without an injury, chest pain, or breathlessness. Also seek prompt help if the foot becomes numb, weak, cold, or unusually pale. A clinician can compare both legs and test muscle and tendon function.
