Inner Shin Pain When Running: What to Know

Short answer

Inner shin pain while running often reflects repeated loading of the muscles and tissues along the shin, but pain that is sharply focused, persists with walking, or worsens despite rest needs medical assessment.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Common trigger
A sudden increase in running load or hills
Pause impact when
Pain occurs during walking or changes your gait
Useful assessment
Tenderness pattern, ankle movement, strength, and training history

What inner shin pain can mean

Pain along the inner border of the shin may develop when running volume, speed, hills, or training surface changes faster than the leg can adapt. The calf and foot muscles attach around this area and help control impact, so fatigue or a sudden increase in demand can make the tissues sore during or after a run. Footwear changes, reduced recovery, and returning to running after a break can add to the load.

The pattern matters. A broad, tender area that eases as activity is reduced can behave differently from pain that can be located with one fingertip. A stress fracture can present with focused shin pain that becomes more noticeable with impact and may continue during ordinary walking. Inner shin pain has several possible explanations, so the symptom alone cannot establish the cause.

What to do now

Pause running and other high-impact exercise while the pain is active, especially sprinting, jumping, and hills. Choose a comfortable low-impact option only if it does not reproduce the pain during the activity or later that day. Keep a brief record of where the pain appears, when it starts, and whether walking or stairs bring it on.

Once everyday walking is comfortable, return gradually with an easy session on a predictable surface. Increase only one training factor at a time, and stop if the pain becomes sharper, spreads, or remains worse afterward. A physiotherapist can assess running technique, calf and foot strength, ankle movement, training changes, and footwear before building a return-to-running plan.

When to seek care

Arrange a clinical assessment if pain is localized to one spot, causes a limp, appears during walking or at rest, wakes you at night, or does not settle after reducing impact. Seek prompt care after a fall or direct blow, or if the lower leg becomes markedly swollen, numb, weak, unusually pale, or cold. Do not test suspected bone pain by repeatedly running on it.

A clinician may examine the tender area, ankle and foot movement, strength, and your running history. Depending on the findings, they may decide whether imaging or a period of protected activity is appropriate. See the guide to [Shin Pain](/symptoms/shin-pain/) for broader symptom information, and learn about [Stress Fracture](/conditions/stress-fracture/) when pain is focal or impact-related.

Questions for your doctor or physiotherapist

Ask: Which structures could be causing this inner shin pain? Does the location or walking pattern suggest that I should avoid all impact for now? How should I adjust my running distance, pace, hills, and surface? What strength or mobility findings should be addressed? What symptoms would mean I need an earlier review?

Questions people ask

Should I keep running with inner shin pain?

Pause running while pain is active, particularly if it alters your stride or continues during walking. Low-impact exercise is reasonable only when it stays comfortable during and after the activity.

How can I tell whether inner shin pain needs assessment?

Arrange an assessment for focused tenderness, pain at rest or during walking, a limp, night pain, swelling, or symptoms that persist after reducing impact.

Can a physiotherapist help with running-related shin pain?

Yes. A physiotherapist can review training changes, running mechanics, calf and foot strength, ankle movement, and a gradual return plan while checking for signs that need medical review.
