What should you know about infected trochanteric bursitis?

Short answer
Infected trochanteric bursitis is an infection near the outer hip that may cause local warmth, redness, swelling, and pain with movement, and it needs prompt medical assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Location
- Bursa beside the outer hip
- Concerning changes
- Warmth, redness, swelling, drainage, or fever
- Activity advice
- Avoid pressure and painful loading until assessed
What it means
The trochanteric bursa reduces friction beside the outer hip. Germs can enter through damaged skin, a wound, an injection, or spread from nearby tissue. Infection may produce focal tenderness, warmth, redness, swelling, and pain when lying on that side or walking. Fever can occur, but its absence does not rule out infection.
Outer-hip pain also has non-infectious causes, including tendon irritation and pressure-related inflammation. A clinician may examine the skin, hip movement, walking pattern, and nearby joints. Tests or fluid sampling may be considered when the diagnosis is uncertain.
What to do now
Arrange prompt medical review rather than treating a hot, red, or draining outer hip as ordinary exercise pain. Avoid pressure on the painful side, pause activities that increase symptoms, and keep any wound clean and covered. Do not inject the area or use leftover antibiotics.
Treatment may involve prescribed antibiotics and drainage when infected fluid has collected. Cortisone Injection is generally not a first step while infection is possible. After infection has settled and a clinician confirms it is appropriate, Physiotherapy may address strength, gait, and load management. Shockwave Therapy should be considered only for a suitable non-infectious problem after assessment.
When to seek care
Seek same-day care for spreading redness, increasing warmth, pus, a new wound, worsening pain, or difficulty bearing weight. Emergency assessment is needed for severe weakness, confusion, fainting, breathing difficulty, or rapidly worsening illness. A painful red area with fever deserves prompt attention because infection can move into surrounding tissues.
Questions for your doctor
Ask whether the bursa, skin, tendon, or hip joint appears involved, whether imaging or fluid testing is needed, and how treatment will be monitored. Clarify which activities are safe, how to protect the skin, and when rehabilitation or a follow-up examination can begin.

Questions people ask
Does outer-hip pain mean the bursa is infected?
No. Outer-hip pain has several possible causes. Infection becomes more concerning when pain occurs with warmth, redness, swelling, drainage, fever, or a nearby wound.
Can physiotherapy treat infected trochanteric bursitis?
Physiotherapy can support recovery after infection has been assessed and treated, but it does not replace antibiotics or drainage when infection is present.
