Infant strep throat: what to know

Strep Throat is an infection caused by streptococcal bacteria. In an infant, possible clues include fever, irritability, crying during swallowing, reduced feeding, drooling, a hoarse cry, or swollen glands. These signs are not specific; a viral illness, Tonsillitis, or another throat problem can look similar.

A clinician may examine the mouth, throat, ears, neck, hydration, and breathing. Testing may be considered when the examination and symptoms support it. A red throat alone cannot confirm Strep Throat, and an infant's age affects how the result and treatment are interpreted.

Offer breast milk or usual formula in smaller, more frequent feeds if swallowing is uncomfortable. Keep track of wet nappies and alertness. Ask a pharmacist or clinician about an age- and weight-appropriate pain or fever medicine; do not use leftover medicines or share antibiotics.

Arrange a GP Consultation if symptoms suggest a significant throat infection, feeding is reduced, or a household contact has confirmed Strep Throat. If Antibiotic Treatment is prescribed, give it exactly as directed and ask what to do if the infant vomits a dose.

Seek urgent help if the infant has difficulty breathing, cannot swallow saliva, has blue or grey lips, is very drowsy, becomes floppy, or shows dehydration such as markedly fewer wet nappies. Get prompt advice for a young infant with a fever, rapidly worsening symptoms, repeated vomiting, or refusal of all feeds.

Ask whether testing is needed, which symptoms are most concerning, and how to maintain hydration. Ask whether the illness is more consistent with Strep Throat or Tonsillitis, how to give any prescribed Antibiotic Treatment, and when follow-up is required.
