What should you know about infant rash on face?

Short answer
An infant facial rash may come from saliva, irritation, eczema, heat, infection, or another condition, and its pattern helps guide assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Common irritants
- Saliva, milk, wiping, products, and heat
- Gentle care
- Lukewarm water, patting dry, plain moisturiser
- Prompt review
- Eye involvement, blisters, crusting, or rapid spread
How facial rashes differ
Rashes around the mouth may follow saliva, milk, wiping, or a product touching the skin. Dry itchy patches on the cheeks can occur with atopic dermatitis, while heat can cause small bumps where the face becomes warm or sweaty. Contact dermatitis may trace a new cream, cleanser, fabric, or detergent. Ring-shaped scaling, crusting, blisters, or a rash that is spreading needs clinical review. Possible causes include [Atopic Dermatitis](/conditions/atopic-dermatitis/), [Contact Dermatitis](/conditions/contact-dermatitis/), [Heat Rash](/conditions/heat-rash/), [Intertrigo](/conditions/intertrigo/), [Ringworm](/conditions/ringworm/), and [Scabies](/conditions/scabies/).
Protect the facial skin
Rinse away milk or saliva with lukewarm water and pat, rather than rub, the area dry. Use a thin layer of plain fragrance-free moisturiser on dry skin, keeping products away from the eyes and mouth. Stop newly introduced cosmetics, essential oils, and fragranced wipes until the cause is clearer. Keep the infant cool, use soft breathable clothing, and avoid picking flakes or spots. Do not use adult acne, steroid, antifungal, or antibiotic products on the face without specific medical advice.
When facial symptoms need help
Arrange an assessment if the rash lasts, spreads, repeatedly returns, or affects the eyelids, eyes, lips, or feeding. Seek prompt help for increasing warmth, pain, swelling, pus, yellow crust, blisters, or rapidly worsening redness. Treat breathing difficulty, swelling of the lips or tongue, marked drowsiness, or an infant who seems seriously unwell as urgent concerns.
What to ask
Ask whether the distribution suggests saliva irritation, eczema, contact dermatitis, heat, or infection; which cleanser and moisturiser are suitable; and whether treatment is safe near the eyes and mouth. Show photographs and mention new products, illness, scratching, discharge, and whether anyone else has a similar rash.
