Infant intertrigo treatment: what parents should know

Intertrigo is irritation in areas where skin touches skin, such as the neck, armpits, groin, or creases around the thighs. Drooling, milk, sweat, urine, stool, and trapped bath water can keep these folds damp. Rubbing then weakens the skin barrier, leaving redness, soreness, or a shiny moist surface. A skin yeast infection can develop in the damaged, warm area; this may cause a brighter red rash with small spots around its edge. A rash can also have another cause, so its appearance alone does not confirm yeast.

A baby may be unsettled when the fold is cleaned, dressed, or touched. This does not tell you which treatment is needed, but it is useful information for a clinician.

Rinse the affected fold with lukewarm water or use a gentle, fragrance-free cleanser, then pat it dry rather than rubbing. Give the fold brief air-drying time during a nappy change or after bathing, while keeping the baby warm. Change wet or soiled nappies promptly and check nearby creases, including the neck and behind the ears. Loose, breathable clothing can limit heat and rubbing.

Do not use talcum powder, fragranced products, essential oils, or leftover prescription creams on an infant’s rash. Powders can be inhaled, and some creams can irritate thin skin or hide a spreading infection. A clinician may recommend a protective barrier product or a medicine such as a topical antifungal treatment when yeast is suspected. Use only the product, amount, and duration advised for your baby’s age and the affected area.

Contact a healthcare professional if the rash is spreading, very painful, cracked, bleeding, blistered, oozing, or developing crusts. Seek prompt advice if your infant has a fever, seems unusually sleepy, feeds poorly, has fewer wet nappies, or is difficult to comfort. Arrange an assessment when gentle skin care does not improve the area, when it keeps returning, or when you are unsure whether the rash is intertrigo, eczema, a bacterial infection, or cutaneous candidiasis.

Do not wait for a routine appointment if your baby appears seriously unwell, has rapidly worsening skin changes, or has swelling around the eyes or mouth.

Ask whether the rash looks like simple moisture irritation or a yeast or bacterial infection, which cleanser and barrier product are suitable, and how often the fold should be checked. Tell the clinician where the rash began, how quickly it changed, whether it affects the mouth or nappy area, and what products have already touched the skin.
