What the rash may represent

Common triggers include heat, saliva, friction, a new detergent, fabric, skincare product or food contact. Dry, itchy patches can occur with eczema or an allergic reaction. A rash from Chickenpox often evolves into fluid-filled spots, while Cellulitis tends to cause a localised area that is hot, painful, swollen or spreading.

The absence of fever is reassuring but does not rule out a condition needing review. Consider when the rash began, where it started, whether it blanches when pressed and whether the baby seems comfortable.

Practical skin care

Keep the skin cool and dry, use loose soft clothing and wash with lukewarm water and a fragrance-free cleanser. Pat rather than rub. Stop any newly introduced product and avoid scratching; keep nails short. Do not apply steroid, antibiotic or herbal creams unless a clinician has recommended the product for your baby.

Take a clear photograph if the rash changes, and record possible exposures such as detergent, clothing, plants or food. Keep the baby hydrated with usual feeds.

When to seek medical help

Seek urgent help if the rash does not fade when pressed, appears with breathing difficulty, facial swelling, repeated vomiting, unusual sleepiness or a baby who is hard to wake. Arrange prompt assessment for rapidly spreading redness, warmth, swelling, blisters, skin peeling, painful sores or a baby who is feeding poorly. A new rash with respiratory symptoms can occur with viral illnesses such as Common Cold or COVID-19 and still warrants attention if the infant seems unwell.

What to ask

Ask whether the rash looks irritant, allergic, infectious or eczema-like, which moisturiser is suitable and whether any product should be stopped. Share photographs, the timing of feeds or product changes and any cough, breathing change or reduced feeding.
