Indigestion Without Eating: What It Can Mean

Short answer
Indigestion without eating can come from stomach irritation, reflux, trapped gas or another upper-digestive problem, so the pattern and accompanying symptoms matter.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What indigestion on an empty stomach can mean
Indigestion, also called dyspepsia, describes discomfort in the upper abdomen rather than a single disease. It can feel like burning, pressure, early fullness, bloating, belching or nausea. Acid can move upward even when no meal has just been eaten; this is associated with [Acid Reflux](/conditions/acid-reflux/) and sometimes a [Hiatus Hernia](/conditions/hiatus-hernia/). Irritation of the stomach lining, known as [Gastritis](/conditions/gastritis/), can also cause symptoms between meals. Other possibilities include [Peptic Ulcer](/conditions/peptic-ulcer/), [Helicobacter Pylori Infection](/conditions/helicobacter-pylori-infection/) or [Gallstones](/conditions/gallstones/). These links describe conditions that can present with indigestion; the symptom alone does not identify its cause.
Notice whether discomfort is worse overnight, after medicines such as anti-inflammatory painkillers, with nausea, or with a sour taste. A symptom diary can help a clinician connect timing, food, medicines and stress with the episodes.
What you can do now
Try smaller, calmer meals when you do eat, and avoid lying down soon after food. Reduce personal triggers such as very fatty or spicy meals, alcohol, smoking and excess caffeine if they worsen symptoms. Take medicines only as directed; a pharmacist can explain whether an antacid or [Acid Reflux Medication](/treatments/medical-management/acid-reflux-medication/) is suitable for you, especially if you take other medicines or are pregnant. Keep a brief record of when symptoms appear, what they feel like and what eases them. Arrange a medical review if episodes keep returning, interfere with sleep or do not settle with simple measures. More general information is available under [Indigestion](/symptoms/indigestion/).
When to seek care
Seek urgent medical help for severe or worsening abdominal pain, vomiting blood, black or tarry stools, fainting, marked weakness, or yellowing of the skin or eyes. Chest pressure, breathlessness, sweating or pain spreading to the arm, jaw or back can sometimes resemble indigestion and needs emergency assessment. Contact a clinician soon if you have repeated vomiting, trouble swallowing, unexplained weight loss, persistent symptoms or a new change in your usual pattern.
Questions for your doctor
Ask what may explain symptoms that occur before meals, whether your medicines could irritate the stomach, and whether testing for Helicobacter pylori or another cause is appropriate. Tell the clinician about pain location, bowel changes, vomiting, appetite, pregnancy possibility and any use of anti-inflammatory painkillers.
