Impetigo in the Diaper Area: What to Know

Diapers create warmth, moisture, and friction, while urine or stool can irritate the skin. Those changes may allow bacteria to enter through tiny breaks. Impetigo can appear as blisters that break easily, shallow erosions, oozing, or crusts. Ordinary irritant dermatitis, yeast infection, eczema, and other rashes can look similar, so a clinician may need to examine the child before treatment is chosen.

Avoid applying leftover antibiotics, steroid creams, or household remedies unless a healthcare professional has advised them for this episode.

Change wet or soiled diapers promptly and clean with lukewarm water or a gentle, fragrance-free cleanser. Pat rather than rub, allow the skin to dry briefly, and use a clean diaper. Wash hands before and after changes, use separate towels, and avoid sharing washcloths. Do not pop blisters or scrape away crusts. A clinician may prescribe a treatment for the skin or medicine by mouth; keep it away from the eyes and use it exactly as directed. Keep the child’s nails short to limit scratching.

Arrange prompt assessment when sores spread beyond the diaper area, become increasingly red, swollen, warm, painful, or drain heavily. Seek urgent help if the child has fever, poor feeding or drinking, fewer wet diapers, marked sleepiness, rapidly worsening skin, or swelling near the eyes. A young infant with a new blistering or crusted rash should be assessed promptly.

Ask whether the rash is impetigo, irritation, yeast, or another condition; which cleanser and treatment are suitable; how to protect caregivers; and which symptoms mean the child needs urgent review.
