Imperforate hymen treatment: what to know

An imperforate hymen is a thin tissue covering that closes the vaginal opening. It is usually present from birth. As puberty progresses, menstrual blood may collect behind the tissue, causing pelvic pressure, cramping, back pain, or a missing first period. Delayed puberty and periods stopped can have many explanations, so these symptoms do not identify one diagnosis by themselves.

A clinician may ask about periods and pain, examine the area, and use a pelvic ultrasound to see whether blood has collected and to check the reproductive organs. The examination can usually be adapted to comfort and age.

Treatment is commonly a minor surgical opening, sometimes called a hymenectomy. The clinician removes or opens the blocking tissue while preserving healthy surrounding tissue. Anaesthesia and the setting depend on age, symptoms, examination findings, and local practice.

Afterward, follow instructions about cleaning, pain relief, activity, and returning for review. Menstrual flow and pressure symptoms often improve once drainage is possible. Do not insert anything into the vagina or resume strenuous activity until the treating team says it is appropriate.

Arrange medical assessment for a missing first period with recurring pelvic pain, a feeling of pressure, difficulty passing urine, or a bulge at the vaginal opening. Seek urgent care for severe or rapidly worsening pain, fainting, fever, vomiting, or inability to pass urine. After treatment, contact the clinic for increasing pain, heavy bleeding, fever, or foul-smelling discharge.

Ask what the examination and pelvic ultrasound show, which procedure is recommended, what anaesthesia will be used, how long recovery may take, and which symptoms should prompt an earlier review.
