What should you know about imiquimod for herpes?

Herpes can cause grouped blisters, shallow sores, burning, tingling, or tenderness, but several skin conditions can look similar. Imiquimod changes local immune activity and may irritate open or inflamed skin. Using it on an unconfirmed sore can delay the right treatment and make the area harder to assess. A clinician may ask about the timing of symptoms, examine the area, and decide whether testing or another medicine is needed.

Do not apply imiquimod to a suspected herpes sore unless a qualified clinician has specifically prescribed it for that situation. Avoid sexual or skin-to-skin contact with the affected area while sores or warning symptoms are present, wash hands after touching the area, and do not share towels or personal items. Keep the skin clean and dry; avoid picking blisters or applying irritating products.

Arrange prompt assessment for a new genital or facial blistering episode, significant pain, spreading sores, fever, difficulty passing urine, or eye pain or redness. Urgent care is needed for eye involvement, confusion, severe illness, or a weakened immune system with new sores. Contact a prescriber if imiquimod has caused marked swelling, ulceration, or worsening pain.

Ask whether the appearance fits herpes or another condition, whether testing is useful, which treatment is appropriate, and how to reduce transmission. Tell the clinician about pregnancy, immune conditions, and any cream already applied.
