How it presents
The iliotibial band runs along the outside of the thigh and crosses the side of the hip. A nearby bursa can become sensitive when repeated activity, direct pressure, or changes in hip muscle control increase local load. Typical symptoms include pain over the bony outer hip, tenderness when lying on that side, and discomfort with stairs, running, or prolonged standing. The condition is also described within the wider group called greater trochanteric pain syndrome. Hip arthritis, tendon pain, and referred pain from the back can overlap with this pattern.
Treatment and self-care
Start by reducing direct pressure and the activity that repeatedly provokes symptoms. Sleep on the less painful side with a pillow between the knees, or lie on your back if that is more comfortable. Physiotherapy can build hip and pelvic strength, improve movement control, and guide a gradual return to activity. If symptoms remain limiting, a clinician may discuss a cortisone injection or shockwave therapy after confirming the likely pain source. Avoid aggressive foam rolling directly over the tender bony area while it is irritable.

Arrange an evaluation if outer-hip pain persists, disturbs sleep, causes a limp, or prevents normal activity. Seek urgent care after a fall when you cannot bear weight, or if the area becomes hot, red, rapidly swollen, or painful with fever.
