Why the stretch may be used

The iliopsoas helps flex the hip and supports movement between the trunk and leg. When the hip stays bent for long periods or repeated activity makes the front of the hip sensitive, a therapist may use a gentle lunge or supported position to explore hip extension. The aim is comfortable movement and better tolerance, not an aggressive pulling sensation. Tightness can coexist with weakness, joint irritation, or a problem elsewhere, so a stretch should follow an examination rather than replace one.

How to practise safely

A therapist may adjust the position of the pelvis, spine, knee, and supporting foot so the movement stays at the front of the hip instead of the lower back. Breathe normally and move slowly into a mild stretch; stop if you feel sharp groin pain, tingling, numbness, or increasing symptoms afterward. Your programme may also include hip and trunk strengthening, movement retraining, and changes to sitting or training habits. The same principles may support rehabilitation for problems such as Achilles tendinitis or an ankle sprain, but the exercises are adapted to the affected area.

When to pause and get help

Contact your therapist or clinician if each session increases pain, walking becomes harder, or symptoms spread down the leg. Seek urgent assessment after an injury that prevents weight-bearing, or if hip pain is accompanied by fever, a hot swollen joint, marked weakness, new numbness, or bladder or bowel changes. A review is also sensible when stretching has not improved function or when pain is present at rest or wakes you repeatedly.
