Understanding the symptom
Cold can reduce pain sensation and calm local irritation. Shoulder pain may involve a tendon, bursa, joint, muscle, or a nerve from the neck. Pain with overhead movement can occur with rotator cuff tendinitis or shoulder impingement, while marked stiffness may occur with frozen shoulder. Relief from ice is useful for comfort but is not a diagnosis.
Using an ice pack safely
Place a cold pack inside a towel and rest it over the painful shoulder for a brief session. Keep the shoulder supported and avoid positions that sharply increase pain. Inspect the skin during and after use; stop for burning, unusual colour, or excessive numbness. Never place ice directly on the skin or leave it on while asleep. Gentle, comfortable movement is usually preferable to holding the arm completely still for long periods.
When assessment is needed
Seek urgent care for shoulder pain after major trauma, a visibly deformed joint, a cold or pale hand, or sudden weakness. Chest discomfort, breathlessness, sweating, or pain spreading to the jaw or arm needs emergency attention. Arrange review for ongoing night pain, loss of strength, increasing stiffness, or difficulty lifting the arm. A clinician can assess for a tear, calcific tendinitis, osteoarthritis, or another cause.
