I feel heartburn: what should I know?

Heartburn often rises behind the breastbone and may leave a sour taste or worsen after meals, bending, or lying down. Acid reflux is a common explanation. Gastritis, a hiatus hernia, obesity, and a peptic ulcer can also be associated with upper abdominal or burning discomfort. Symptoms vary, and heartburn does not confirm any one condition. Note the timing, food or drink triggers, relation to lying down, and whether swallowing is painful or difficult.

Try smaller meals, avoid lying down soon after eating, and identify foods or drinks that reliably trigger symptoms. Raise the upper part of your sleeping surface if night symptoms occur, rather than relying only on extra pillows. Avoid smoking and limit alcohol if they worsen the burning. A pharmacist can advise whether an antacid or acid reflux medicine is suitable, including checks for pregnancy, kidney disease, and interactions. Do not take frequent over-the-counter treatment indefinitely without review.

Call emergency services for chest pressure or pain with breathlessness, sweating, faintness, nausea, or pain spreading to the arm, jaw, back, or shoulder. Arrange medical assessment for difficulty or pain with swallowing, vomiting blood, black stools, persistent vomiting, unexplained weight loss, or symptoms that keep returning despite appropriate advice. A clinician can distinguish reflux from heart, stomach, or food-pipe problems.
