The short answer

Painful lifting is a symptom rather than a diagnosis. Rotator cuff tendinitis, impingement, calcific tendinitis, frozen shoulder, and shoulder arthritis can limit overhead movement. A rotator cuff tear may cause sudden weakness after an injury. Pain that travels below the elbow, with tingling or neck pain, may come from cervical radiculopathy rather than the shoulder itself.

What the movement pattern can tell you

Pain mainly during reaching overhead can occur when shoulder tendons are irritated. Marked stiffness in several directions, including reaching behind your back, can fit a frozen shoulder. A painful catching sensation or weakness when lifting may need assessment of the rotator cuff. Sudden loss of power after a fall, or an inability to raise the arm at all, is more concerning than gradual soreness after activity.

Tell the clinician whether symptoms began after an injury, disturb sleep, radiate into the arm, or come with numbness. These details help separate a shoulder structure from a neck nerve or joint problem.

What to do now

Avoid repeated overhead lifting and heavy pushing while keeping the elbow and shoulder moving gently within a comfortable range. Support the arm when resting, use a wrapped cold pack for a recent flare, and consider simple pain relief only if it is suitable for your health and medicines. Do not force stretches through sharp pain or repeatedly test a weak shoulder.

Book an assessment if pain persists, limits dressing or sleep, or keeps returning. Examination may include strength and range-of-motion testing; imaging is chosen when the history and examination suggest a tendon tear, arthritis, calcific deposit, or another cause.

When to seek medical care

Seek urgent care after a fall or heavy lift if the shoulder is visibly deformed, suddenly weak, or impossible to move. Get emergency help for shoulder or arm pain with chest pressure, breathlessness, sweating, or faintness. Prompt assessment is also needed for fever with a hot swollen joint, spreading redness, or new arm numbness and weakness.
