Why spotting can happen with Mirena

Mirena releases a progestogen inside the womb, which can make the lining thin and cause light, irregular bleeding, especially after insertion or a change in bleeding pattern. Spotting can also reflect pregnancy, an infection such as chlamydia, cervical dysplasia, polyps, fibroids, or another problem affecting the cervix or womb. The device can occasionally move, so a changed thread length or new pelvic discomfort matters.

What to monitor and do

Keep a calendar of spotting, heavier bleeding, pain, discharge, fever, and any change in the threads. Use products that allow you to see the amount of blood and avoid inserting anything into the vagina if it causes pain. Do not remove the device yourself or take hormones, anti-inflammatory medicines, or herbal products without checking suitability. A clinician can examine you, check pregnancy or infection when needed, and discuss options if the bleeding remains unacceptable.
