How Long Does Sports Physiotherapy Take to Work for an ACL Tear?

Short answer
Sports physiotherapy starts improving movement and strength over staged appointments, while return to cutting or pivoting sports after an ACL tear takes a carefully assessed rehabilitation period.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Early targets
- Control swelling, restore movement, and walk safely.
- Later targets
- Build strength, landing control, and confidence before pivoting sport.
Why ACL rehabilitation is staged
The anterior cruciate ligament helps control forward movement and rotation of the knee. After a tear, physiotherapy may first target swelling, full knee straightening, bending, and safe walking. Later work develops thigh and hip strength, balance, landing control, and responses to direction changes. Some people follow rehabilitation before surgery, while others complete it before and after reconstruction; the plan also depends on other injured knee structures and sporting goals.
Make each phase count
Attend reviews so exercises can be progressed when the knee is calm and movement is improving. Practise the prescribed programme with careful technique, and avoid pivoting, jumping, or return-to-sport tests before clearance. Record swelling, giving-way episodes, confidence on stairs, and how the knee responds the following day. A brace, walking aid, or modified training may be appropriate for a period decided by your clinician.
When to contact your care team
Contact your clinician promptly after a new giving-way episode, a locked knee, rapidly increasing swelling, severe pain, fever, wound changes after surgery, or calf swelling with chest symptoms. Arrange reassessment when progress stalls, exercises repeatedly provoke swelling, or you cannot achieve the movement and control targets set for your phase.
Questions for your team
Ask which rehabilitation phase you are in, whether surgery is being considered, and which strength, balance, and hopping checks are required before sport. Clarify how to modify work or training and what knee swelling after exercise means for the next session.

Questions people ask
Will physiotherapy heal a torn ACL?
Physiotherapy can improve strength, movement, control, and function; whether surgery is needed depends on the knee, associated injuries, and activity goals.
Can I return to sport when the knee feels better?
Feeling better is only one sign; clinicians also assess strength, control, swelling, and sport-specific movement before clearance.
