How long does rotator cuff repair take to work for a rotator cuff tear?

Short answer

Rotator cuff repair often needs several months before the shoulder feels substantially more useful, because the tendon must heal before strengthening can progress.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Progression
Supported movement comes before active lifting and strengthening

What the timeline means

After repair, the shoulder is usually protected while the tendon attaches and the incision settles. Early physiotherapy may concentrate on supported or passive movement, with active lifting and resistance introduced later. This can feel slow, especially when pain has already eased, but loading the tendon too soon can disrupt recovery.

The pace varies with tear size and location, tissue quality, the repair performed, smoking or other health factors, and whether stiffness or weakness was present before surgery. Everyday use normally returns in stages; overhead work, heavy lifting and sport may require a longer progression.

How to support healing

Wear the sling for the period prescribed and do only the exercises approved for your stage. Avoid lifting with the repaired arm, sudden reaching and pushing yourself up from a chair unless your surgeon has cleared these actions. Attend therapy and use changes in pain, movement and strength to guide progression rather than testing the shoulder repeatedly.

When to seek care

Contact your surgical team for increasing wound redness, drainage, fever, uncontrolled pain, new numbness or a sudden loss of the movement you had regained. Urgent assessment is needed for severe breathing difficulty, chest pain, or a hand that becomes cold or discoloured.

Questions for your surgeon or therapist

Ask when active shoulder movement and resistance exercises can begin, which sleeping position is safest, and when you may drive, work or lift. Ask how the repair will be checked and how to distinguish normal exercise soreness from a warning sign.

Questions people ask

Why can the shoulder hurt after repair even when healing is progressing?

The operation and rehabilitation can cause temporary soreness and stiffness, while the tendon and surrounding tissues recover.

Can I strengthen the shoulder as soon as pain improves?

No. The surgeon’s staged plan matters because pain relief does not prove that the tendon is ready for resistance.
