Some people notice a small shift after an initial conversation, such as having a plan for a difficult evening. For others, progress is gradual or changes when treatment, scan results, sleep or family pressures change.

The first sessions usually clarify what is troubling you, what support you want and whether a particular approach fits. Your counsellor may use talking therapy, practical coping exercises, relaxation, problem-solving or support for communication. A review looks at your goals and functioning rather than expecting every difficult feeling to disappear.

How to make counselling useful

Before each session, note one situation that felt hard, how you responded and what you would like to handle differently. Be open about sleep, appetite, panic, concentration, treatment worries and any medicines or other support you use. Between sessions, practise one agreed strategy and tell the counsellor what helped or felt unsuitable.

If your mood is not improving, or the counselling relationship does not feel right, say so. The plan may be adjusted, sessions may be spaced differently, or your oncology team may arrange a medical review for symptoms that need additional support.

When to seek care

Contact your care team promptly if anxiety or low mood is stopping you from eating, sleeping, attending treatment or managing essential tasks. Seek urgent help if you feel unable to stay safe, have thoughts of suicide or self-harm, or cannot care for yourself. Tell someone you trust and use local emergency services or the crisis route supplied by your treatment centre.

Ask: What type of counselling is available for my concerns? How will we measure progress? What can I practise between sessions? Could pain, medication, sleep or another health problem be affecting my mood? Who do I contact urgently if I feel unsafe?
