How long does osteotomy take to work for Perthes disease?

Perthes disease affects the developing hip, so treatment aims to maintain a useful relationship between the ball and socket while the hip changes over time. An osteotomy repositions bone to improve containment or alignment. That position is established during the operation, but soreness, weakness and restricted movement can continue during early recovery.

The treating team follows hip motion, walking, comfort and imaging. The child’s age, stage of Perthes disease, operation performed and need for additional treatment all influence the pace. A change in the X-ray and a change in everyday movement may occur at different times.

Follow instructions about casts, braces, crutches, positioning and weight-bearing. Give medicines only as directed, check the incision or cast as advised, and keep scheduled reviews so hip position and bone healing can be monitored.

Therapy may begin with gentle range-of-motion work and safe transfers, then move towards muscle strength, balance and age-appropriate walking. Keep play within the agreed limits, even when the child feels energetic. Tell the team if exercises cause persistent pain, guarding or a clear decline in walking.

Contact the care team for increasing pain, fever, wound drainage, spreading redness, a tight or damaged cast, new numbness, or toes that become pale, blue or cold. Seek urgent care for severe breathlessness, chest pain, or sudden swelling and pain in a calf.

Ask what the osteotomy is intended to improve, how hip containment will be checked, how long restrictions may last, and which positions or activities should be avoided. Ask when school, swimming, walking outdoors and sport can be reintroduced.
