How long does JAK inhibitor treatment take to work for vitiligo?

The medicine changes immune signalling involved in vitiligo, but pigment-producing cells need time to become active and repopulate lighter skin. Facial areas may respond before hands, feet, or other areas with fewer pigment cells. Early signs can be tiny brown or tan spots that gradually join together rather than an immediate return of colour.

A dermatologist may assess photographs, the edges of patches, and new areas of depigmentation during follow-up. Lack of visible change early in treatment does not by itself show that the plan will not work.

Apply or take the medicine exactly as prescribed and keep follow-up appointments, even when improvement is subtle. Tell your dermatologist about other medicines, infections, pregnancy, or plans for pregnancy before starting or changing treatment. Protect depigmented skin from sunburn, because it has less natural protection and contrast can make patches more noticeable.

Do not increase the amount or frequency to speed repigmentation. Keep dated photos in similar lighting so gradual changes are easier to compare, and report new patches or irritation rather than stopping without advice.

Seek medical advice promptly for spreading redness, marked swelling, blistering, severe pain, pus, or a rapidly worsening rash at treated sites. Contact your clinician urgently for breathing difficulty, facial swelling, or widespread hives. Arrange a routine review if you see no meaningful change after the period your dermatologist set, or if side effects make regular treatment difficult.

Ask which areas are realistic targets, when your first progress review should occur, and what early repigmentation will look like for your skin. Clarify how long to continue before judging the response, whether light treatment is part of the plan, and which symptoms should prompt a pause or urgent call.
