How long does immunotherapy take to work for oesophageal cancer?

Immunotherapy supports an immune response against cancer cells, and that response does not follow an identical timetable for everyone. Oesophageal cancer treatment may be influenced by the tumour location, stage, biopsy results, immune markers and any chemotherapy or radiotherapy given with it. Trouble swallowing can have more than one cause, including the tumour, treatment irritation or narrowing of the food pipe, so a symptom alone cannot confirm whether treatment is working. Your team may use scans, endoscopy when appropriate, weight, hydration, eating ability and examination findings to build a clearer picture.

Keep every scheduled review and tell your team whether food and fluids are passing more easily, staying the same or becoming harder to swallow. Mention coughing during meals, vomiting, pain behind the breastbone, reduced intake and changes in weight or energy. Follow the eating and drinking advice provided by your care team, and ask whether a dietitian or swallowing specialist would help. Before treatment starts, ask when imaging or other checks are planned and how an uncertain result will be followed up.

Contact your cancer team promptly if swallowing becomes suddenly worse, you cannot keep fluids down, you are passing very little urine, or you develop persistent fever, severe diarrhoea, yellowing of the skin or eyes, or a new breathing problem. These can reflect dehydration, obstruction, infection or an immune-related treatment effect and should be assessed. Seek urgent help for choking, severe breathlessness, fainting or vomiting blood. Urgency is based on the symptom and its severity, not on whether it indicates treatment response.
