How long does immunotherapy take to work for Merkel cell carcinoma?

Immunotherapy helps the immune system recognise and attack cancer cells. Some people notice changes in symptoms or tumour size during treatment, while others need more time before a response is clear. Merkel cell carcinoma can sometimes show an early change that needs careful interpretation, so one scan or one difficult day does not always show the final treatment effect.

Your team may compare your examination, symptoms, blood tests and imaging with your starting results. The interval between assessments depends on your medicine, treatment plan, cancer extent and general health. Treatment may continue when scans show stability or improvement, provided the benefits outweigh concerns about side effects.

Attend treatment and review appointments as planned, and keep a short record of new symptoms, their severity and when they began. Tell the oncology team about changes even if they seem unrelated to the cancer, because immune treatment can affect organs such as the skin, bowel, lungs, liver, thyroid and other hormone-producing glands.

Do not stop, delay or restart treatment without speaking with your oncology team. Ask when your next assessment is expected and which changes should be reported between visits. A delayed response or a treatment adjustment does not by itself show that future options have run out.

Contact your oncology team promptly for new or worsening breathlessness, persistent cough, severe diarrhoea, blood in stool, marked abdominal pain, yellowing of the skin or eyes, extensive rash, severe weakness, confusion, or a sudden change in vision. Seek emergency help for severe breathing difficulty, fainting, chest pain or symptoms that rapidly worsen. These problems can reflect immune-related inflammation and may need assessment before the next appointment.

Ask which findings will show that treatment is helping, when imaging or another assessment is planned, and how stable disease will affect the next decision. You can also ask which symptoms require a same-day call and whether any medicines or supplements could interfere with your treatment plan.
