How long does imiquimod treatment take to work for actinic keratosis?

Imiquimod stimulates an immune response in the treated area rather than removing the spot immediately. The skin may become red, dry, crusted, swollen, or tender during treatment. This reaction can be part of the expected response, but its intensity does not by itself prove that every abnormal cell has cleared.

The schedule depends on the strength prescribed, the body area, and whether one or several areas are being treated. Healing can continue after the final application, so the clinician may arrange a review once the inflammation has eased.

Apply a thin layer exactly as directed to clean, dry skin and wash your hands afterwards. Do not add extra applications to speed the result. Protect the area from sun exposure, avoid picking crusts, and use only products your clinician says are suitable while the skin is reactive.

Keep a note of application days and changes such as increasing soreness, discharge, or spreading irritation. If you miss an application or the reaction makes normal activities difficult, contact the prescribing clinic for specific instructions rather than changing the schedule yourself.

Contact your clinician if the treated area becomes markedly painful, develops pus, forms extensive blistering, or does not begin settling after the planned course. Seek prompt assessment for a spot that bleeds repeatedly, grows quickly, develops a firm lump, or remains visibly concerning after healing, because not every persistent lesion is actinic keratosis.

Ask which days to apply imiquimod, when to pause for irritation, how long healing may take after the last dose, and when the treated area should be checked. Clarify what to use for washing and moisturising, and what changes should trigger an earlier appointment.
