How long does hot and cold therapy take to work for ankle sprain?

Short answer
Cold therapy may ease ankle-sprain pain and swelling soon after application, while heat may loosen stiffness later; neither repairs the ligament by itself.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Cold
- May give short-term relief for pain and swelling after application.
- Heat
- May ease stiffness when acute swelling has settled and movement is limited by tightness.
What hot and cold can do
Cold can temporarily reduce pain signals and limit the feeling of swelling around a recently injured ankle. Heat increases the sense of looseness in stiff muscles and may be more comfortable before gentle movement once the acute swelling has settled. These effects are temporary. The ligament heals through time, suitable loading, balance work, and progressive strengthening, so symptom relief does not prove the ankle is ready for sport.
Using therapy safely
Wrap a cold pack or warm pack in a cloth and check the skin often. Stop if the skin becomes very pale, numb, blistered, or unusually painful. Avoid heat over a hot, freshly swollen ankle, and do not sleep with either pack in place. Combine symptom relief with comfortable ankle movements, elevation, and a gradual return to weight bearing as advised.
When to seek care
Arrange medical assessment if you cannot take a few steps, the ankle looks deformed, pain is sharply focused over a bone, numbness develops, or swelling and bruising keep worsening. Urgent care is needed for a cold or pale foot, severe increasing pain, or loss of circulation or sensation.
Questions for your doctor
Ask whether your injury needs imaging, when heat is appropriate, how to protect the ankle while walking, and which balance and strengthening exercises fit your stage of recovery.

Questions people ask
Can hot and cold therapy heal a sprained ligament?
It can make symptoms more manageable, but ligament healing depends on protection, progressive movement, and rehabilitation.
Which should I use first?
Cold is commonly more suitable for a recently swollen ankle; heat may be useful later for stiffness, provided it does not increase swelling.
