Paget's disease causes areas of bone to remodel more actively than usual. A bisphosphonate reduces this activity; it does not instantly reshape bone that has already changed. Your clinician may use symptoms, examination, and blood tests such as alkaline phosphatase to judge response. The timing depends on the medicine used, the bones involved, and your starting results.

Take the medicine exactly as prescribed and follow instructions about fluids, posture, or fasting when applicable. Keep follow-up appointments so your response and kidney health can be reviewed.
