How long does allogeneic stem cell transplant take to work for myelofibrosis?

After conditioning treatment, the donated cells are infused and travel to the bone marrow. Engraftment is the stage when they start making new blood cells. During this early period, low counts can cause tiredness, infection risk or bleeding, so monitoring is close. The transplant team also watches for donor-cell activity that can help control myelofibrosis. Spleen size, symptoms, blood counts and marrow findings may change at different speeds; one early result does not capture the full response.

Attend every blood test and clinic review, take medicines exactly as prescribed, and follow food, hygiene and activity guidance from the transplant unit. Record fever, bleeding, new pain, breathlessness, diarrhoea, rash or changes in the spleen-related fullness. Do not stop immune-suppressing or preventive medicines without the team’s advice. Ask which results will show engraftment and when the team plans marrow or imaging assessments.

Contact the transplant team promptly for a fever or chills, new shortness of breath, confusion, uncontrolled bleeding, severe diarrhoea, repeated vomiting, or a rapidly spreading rash. These symptoms can signal infection, bleeding or graft-versus-host disease and need timely assessment, even if the next appointment is soon.
