How is radiating leg pain treated?

Pain travelling from the back or buttock into the leg can occur when a spinal nerve is irritated or compressed. Sciatica, spinal stenosis, spondylolisthesis and piriformis syndrome are possible causes, but the pattern of pain alone cannot establish which one is present. A clinician may check strength, sensation, reflexes, walking and back movement before deciding whether imaging is useful.

Early care may include activity modification, a gradual return to movement and medicines selected for your health history. Non-surgical spinal decompression may be discussed in some care plans, but treatment should match the suspected source rather than the pain label alone.

Avoid prolonged bed rest and change position regularly, while temporarily reducing movements that sharply worsen the pain. A physiotherapist or clinician can guide exercises for the back, hip and leg and explain safe lifting. An epidural steroid injection may be considered for selected nerve-root pain. Microdiscectomy is a surgical option when a disc problem continues to compress a nerve despite appropriate care or causes significant neurological symptoms.

Seek emergency assessment for new loss of bladder or bowel control, numbness around the inner thighs or groin, or rapidly worsening weakness in a leg. Seek prompt review for foot drop, repeated falls, severe pain after major injury, fever with back pain, or unexplained weight loss. These features require examination because nerve compression and other serious causes need timely decisions.

Ask which nerve or structure may be involved, what changes in strength or sensation to monitor, which movements are safe, and when an injection, scan or surgical opinion would be appropriate.
