Why treatment is gradual

Frozen shoulder causes pain and restriction because the tissues around the shoulder joint become tight and movement becomes limited. Pain may be worse at night, while everyday tasks such as dressing, reaching overhead or fastening clothing become difficult. A clinician checks active and assisted movement and considers other causes of shoulder pain.

The plan changes with the painful, stiff and recovery phases. Forcing a painful stretch can irritate the shoulder, so exercises are usually gentle, consistent and matched to what the joint can tolerate.

What treatment may include

A physiotherapist can teach controlled range-of-motion exercises, posture and shoulder-blade work, then progress strengthening as movement returns. Heat or hydrotherapy may make movement more comfortable for some people. A clinician may recommend suitable pain relief or a cortisone injection when pain is preventing participation in rehabilitation.

Keep using the arm within a tolerable range, but avoid repeated forceful stretching or heavy lifting during a painful flare. If substantial stiffness continues despite an appropriate course of non-surgical care, shoulder arthroscopy may be discussed to release tight tissue.

When to seek care

Seek prompt assessment after a fall or other injury, or if the shoulder becomes suddenly very painful, visibly deformed, hot and swollen, or associated with fever. New arm weakness, spreading numbness, chest discomfort or breathing difficulty needs urgent medical attention. Arrange review when pain or reduced movement is worsening or stopping sleep and daily activities.
