Why treatment differs

The anterior cruciate ligament helps control forward movement and rotation of the knee. Treatment decisions consider whether the tear is partial or complete, whether the knee gives way, associated meniscus or cartilage injury, physical demands and personal goals. Examination and imaging help define the injury, but the scan is considered alongside how the knee functions.

Some people manage well with sports physiotherapy, strength work, movement retraining and activity changes. Others may need ACL reconstruction to restore stability for demanding pivoting activities or when repeated giving-way threatens other knee structures. Surgery is followed by a substantial rehabilitation programme rather than being the whole treatment.

Steps in recovery

Early care aims to settle swelling, restore comfortable knee straightening and bending, and maintain safe leg strength. Prehabilitation before reconstruction can improve movement and strength and helps prepare the knee for rehabilitation. A brace may be useful for selected activities or instability, but it does not replace a stability assessment.

After surgery or a non-operative decision, rehabilitation progresses through strength, balance, landing and change-of-direction work. Return to sport should be based on movement quality, strength, confidence and the clinician’s testing rather than a calendar date.

When to seek care promptly

Arrange prompt assessment after a knee injury with marked swelling, inability to straighten the knee, repeated giving-way, or inability to bear weight. Urgent care is needed if the foot becomes cold, pale or numb, or if severe pain follows a new injury. Worsening swelling, redness or fever during recovery also needs medical review.
