How effective is tumour removal surgery for Merkel cell carcinoma?

Merkel cell carcinoma is an uncommon skin cancer that can grow and spread beyond the original skin lesion. Surgery usually focuses on removing the visible tumour with a surrounding margin and checking nearby lymph nodes when indicated. The skin defect may need closure, a graft or another reconstructive approach.

The pathology report examines the tumour edge and lymph nodes. Findings such as tumour depth, involved nodes or disease beyond the original site may lead to radiotherapy, immunotherapy or closer surveillance. Surgery can be one part of treatment rather than the entire plan.

Ask which skin area and lymph nodes will be treated, whether reconstruction is expected, and how the result will be checked. Protect the healing wound as instructed and keep scheduled skin and lymph-node examinations. Tell the team about medicines that affect bleeding and any new or changing skin lesions.

Call the treating team for spreading redness, warmth, pus, fever, increasing pain, wound separation or new swelling near the operation site. Seek emergency help for uncontrolled bleeding, fainting, severe breathing difficulty or sudden facial or neck swelling.

What margin is planned around the skin tumour? Do I need lymph-node sampling or removal? Will reconstruction or radiotherapy be part of treatment? How often will my skin and nearby lymph nodes be examined?
