How effective are topical corticosteroids for nummular eczema?

Nummular eczema, also called discoid eczema, produces round or coin-shaped patches that may be dry, scaly, cracked or weepy. A topical corticosteroid reduces the local inflammatory response, which can ease itch and redness and make the patch less active. The medicine does not replace moisturising, and it may not be the right choice if the patch is infected or the diagnosis is uncertain. Thick scale or intense dryness can also make a clinician adjust the plan.

Follow the prescribed strength and timing, applying a thin layer to the eczema rather than unaffected skin. Use a fragrance-free moisturiser regularly to support the barrier, with timing guided by your clinician or product instructions. Keep showers comfortably warm rather than hot, use gentle cleansers, and avoid wool, fragranced products and other known irritants. Do not scratch or seal medication under a dressing unless told to do so. If several patches are present, ask how much product is appropriate before treating them all.

Arrange an appointment if patches do not improve, keep appearing, or look different from one another. Pain, spreading redness, warmth, pus, honey-coloured crusts or fever can indicate infection and need assessment. Seek urgent help for rapidly developing facial swelling, breathing difficulty or a severe widespread blistering reaction.
