How effective are topical corticosteroids for lichen simplex chronicus?

Lichen simplex chronicus is skin that has become thick, leathery and itchy after repeated rubbing or scratching. A prescribed corticosteroid lowers local inflammation, which can make the itch less intense and give the surface time to heal. It does not remove the trigger, such as friction, an irritating product, stress-related scratching or an underlying itchy skin problem. As the itch settles, the scratch cycle may become easier to break.

Use the product, amount and schedule prescribed for the affected patch. Apply it to the involved skin rather than spreading it broadly, and wash your hands afterward unless your hands are being treated. A bland moisturiser can support the skin barrier, often when the steroid has been absorbed or at a separate time if advised. Keep nails short, cover or protect areas rubbed during sleep, and avoid friction from rough clothing. Do not continue treatment longer than directed, especially on thin skin such as the face, groin or skin folds.

Arrange a review if the patch is not settling, repeatedly returns, spreads, becomes painful, or develops crusting, oozing or warmth. These changes can signal infection or a different diagnosis. Seek urgent help for rapidly spreading swelling, facial or eye involvement, breathing difficulty, or feeling acutely unwell. A clinician may need to check the itch trigger and adjust the treatment rather than simply extending the steroid.
