How effective are topical corticosteroids for guttate psoriasis?

Guttate psoriasis produces many small, scaly spots that may appear on the trunk and limbs. A topical corticosteroid suppresses inflammation in the skin and can make individual spots less raised, itchy and scaly. Applying cream over numerous areas can be difficult, and a short-lived response may occur if the underlying flare continues. A recent sore throat or other infection can be relevant, so the rash should be assessed rather than treated as ordinary irritation.

Use the prescribed strength and schedule, spreading a thin layer over the affected plaques rather than rubbing harshly. Moisturiser can reduce scaling and support comfort when used as advised. Do not apply stronger medicine more often or over larger areas without review. Keep a record of recent illness, medicines and changes in the rash. If many spots are involved, ask whether a treatment plan covering the whole affected area would be more practical.

Seek prompt care if the rash is rapidly spreading, painful, associated with fever, or producing pus. Urgent assessment is needed for widespread redness with tenderness, extensive peeling, severe illness or a sudden change affecting most of the skin. Arrange review when joint pain, repeated flares or persistent spots develop, or when treatment causes burning, thinning or unusual bruising.
