How effective are topical calcineurin inhibitors for vitiligo?

Vitiligo occurs when pigment-producing cells are lost or become inactive in affected skin. Calcineurin inhibitors reduce local immune activity, which may give pigment cells an opportunity to return or function, particularly where follicles can contribute pigment. Colour often returns unevenly, beginning around hair follicles or patch edges, and hands, feet and areas with little follicular pigment can be harder to treat. The medicine is not a quick cosmetic cover and does not prevent every new patch. A dermatologist may combine it with light-based treatment or another approach when appropriate.

Apply only to the areas and schedule prescribed, avoiding eyes and mucous membranes. Protect depigmented skin from ultraviolet exposure with clothing, shade and sunscreen, since it burns more easily and tanning of nearby skin can increase contrast. Use gentle skin care and avoid picking, friction or unregulated pigment products. Take comparison photographs under similar lighting and allow time for assessment; day-to-day colour changes can be misleading. Tell your clinician about new patches, irritation and any treatment used elsewhere on the body.

Arrange review if patches are rapidly expanding, appearing in several new areas, or affecting the eyes, lips, hands or genital skin. Seek advice for painful redness, blistering, marked swelling or persistent burning after application. A new light patch with scale, itch or a changed surface should be checked because not every pale area is vitiligo. Emotional distress, bullying or avoidance of daily activities is also a valid reason to ask for support.

Ask whether the diagnosis is confirmed, which sites are most likely to respond, how progress will be photographed, and whether combination treatment is suitable. Discuss sun protection, maintenance after colour returns and what to do if new patches appear.
