A gel filler can add support beneath a hollow, making the transition between the lower eyelid and cheek look smoother. It may help when shadowing is mainly caused by loss of volume or a natural groove. It is less likely to satisfy someone whose main concern is puffiness, loose skin, prominent fat pockets, pigmentation or fluid-related swelling.

The under-eye is a delicate area. Swelling, bruising, lumps, bluish colour or unevenness can occur, and filler may remain visible or require correction. A consultation should include an examination of the eyelid and cheek, not just a photograph.

Choose a qualified medical injector who regularly treats the under-eye and can explain why filler is appropriate for your anatomy. Share allergies, previous filler, eye conditions, medicines and any tendency to swell. Ask what product is planned, how much correction is intended and how an unwanted result could be managed.

Follow instructions about medicines and skin care, and arrange the procedure when you can allow swelling to settle. Do not massage a treated area unless the injector specifically tells you to. Protect the skin from sun while bruising or inflammation is present.

Seek emergency medical help for sudden vision change or loss, severe eye pain, unusual headache, weakness, difficulty speaking, or skin that becomes pale, dusky or increasingly painful. Promptly contact the clinic for rapidly increasing swelling, marked redness, discharge or fever. Serious complications are uncommon but need immediate assessment.

Ask whether your hollow is caused mainly by volume loss, shadowing or another feature; what improvement is realistic; what alternatives exist; how long swelling may last; and what emergency pathway is available if vision or severe pain changes.
