How effective is subacromial decompression for shoulder impingement?

Subacromial decompression is an arthroscopic procedure intended to create more room around structures beneath the acromion. A surgeon may remove inflamed tissue or reshape a small part of bone when clinical findings support that approach. Shoulder pain can also arise from rotator-cuff disease, frozen shoulder, arthritis or neck-related nerve irritation, and decompression does not correct every cause. The decision therefore depends on examination, imaging when useful, strength, movement restriction and how symptoms respond to physiotherapy or other non-surgical measures.

Discuss the suspected pain source, the expected change and alternatives with your shoulder specialist. After surgery, rehabilitation usually restores comfortable movement first, then shoulder-blade control and strength. Follow wound, sling and activity instructions, and avoid testing the shoulder with heavy lifting before clearance. Improvement is assessed over recovery rather than immediately after the procedure. Keeping exercises regular and reporting a stalled recovery helps the team adjust the programme.
