Why the decision is individual

A hypertrophic scar is thick and raised but generally stays within the original wound boundary. It may itch, feel firm, limit movement or remain noticeable because of its colour and height. Excision can remove the bulky portion, while rearrangement or release may reduce tension and improve function. However, the incision is another controlled injury, and the healing response can produce a new raised scar.

The surgeon considers how long the scar has been present, whether it is still changing, its location, tension, symptoms and your personal history of raised scars. Injections, silicone-based care, pressure or other treatments may be used before or alongside surgery.

Steps before and after treatment

Have the scar assessed rather than choosing surgery from its appearance alone. Describe itching, pain, tightness and movement restriction, and disclose previous scar treatments, medicines, allergies and any history of keloid or hypertrophic scarring. Ask how recurrence would be prevented and whether a less invasive option could meet your goal.

After surgery, keep dressings and the incision as instructed, avoid pulling or rubbing the area, and attend follow-up visits. Sun protection can help reduce persistent colour contrast once the wound has healed enough for it. Report changes early instead of treating a raised area on your own.

When to seek care

Seek review if the incision opens, bleeds persistently, becomes increasingly painful or warm, drains fluid, or is accompanied by fever. Ask for assessment if a new ridge becomes thicker, itchy or tight after healing. Urgent care is appropriate for severe swelling, breathing difficulty or a rapidly spreading reaction.
