How effective is prehabilitation for ACL tear?

What prehabilitation does
An ACL tear can cause swelling, loss of full knee extension, thigh weakness and a feeling that the knee may give way. Prehabilitation addresses these issues before surgery or a structured non-surgical programme. It may include swelling management, gentle range-of-motion work, quadriceps activation, hip strength, balance and walking practice.
A calmer, more mobile knee gives rehabilitation a useful starting point, but prehabilitation does not reconnect a torn ligament. The programme may need to change when there is a meniscus injury, major instability, locking or pain that limits exercise.
How to prepare effectively
Work with a physiotherapist to choose exercises that fit your knee’s stability and your daily demands. Focus on regaining comfortable straightening, activating the thigh and controlling swelling rather than testing the knee with cutting, pivoting or jumping. Use any brace or walking aid as instructed.
Practise the transfers, crutch technique and home exercises that may be useful later. Tell the clinician if the knee locks, repeatedly gives way, swells substantially after activity or cannot straighten; these details can change the plan.
When to seek medical care
Arrange prompt assessment for a locked knee, rapidly increasing swelling, inability to bear the permitted weight, new calf swelling or worsening weakness. Seek urgent help for severe calf pain with breathlessness or chest pain, or a foot that becomes cold, pale or numb.
