How effective is post-operative rehabilitation for patellar tendon rupture?

Short answer

Post-operative rehabilitation can effectively improve knee movement, quadriceps control and walking after patellar tendon repair, with loading increased only as healing permits.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What recovery usually focuses on

The patellar tendon connects the kneecap to the shin and helps the quadriceps straighten the knee. After repair, the knee is commonly protected while the tendon heals. Rehabilitation then uses carefully selected movement, swelling management, thigh-muscle activation, gait practice and later strengthening to rebuild that chain of control.

Progress is not simply about bending the knee as far as possible. The therapist also watches whether the leg can support the body, whether the kneecap and knee move in a controlled way, and whether daily tasks can be completed without a compensating limp.

Practical steps during rehabilitation

Keep the brace position, walking instructions and exercise limits set by the surgeon. Use crutches or another aid until the care team confirms that the quadriceps can control the knee safely. Practise the prescribed muscle-setting and movement exercises without forcing the knee, and use the recommended approach for swelling.

Avoid deep squats, sudden lunges, jumping and unapproved resistance while the repair is vulnerable. Tell the therapist about buckling, a persistent limp, difficulty lifting the leg, or pain that changes the way you move so the programme can be adjusted.

When to contact the surgical team

Get prompt advice for increasing wound redness, drainage, fever, rapidly worsening swelling, or pain that is not settling. An urgent assessment is needed after a fall, a sudden pop, a new gap around the kneecap, inability to straighten the knee or lift the leg, or a sudden loss of walking control. Chest pain or breathing difficulty needs emergency help.

Questions for your rehabilitation team

Ask when the brace angle may change, how much weight-bearing is allowed, which exercises activate the quadriceps safely, and what signs indicate readiness for less support. Ask when stairs, cycling, running or work duties can be considered.

Questions people ask

Why is knee bending increased gradually?

A repaired patellar tendon needs time to tolerate tension, so movement is advanced in a controlled way rather than forced early.

What does knee buckling during recovery mean?

Buckling can reflect reduced quadriceps control or another problem; report it promptly so walking support and exercises can be reviewed.
