The goal is a shoulder that tolerates useful activity better, not simply a different resting position. Improvement may be gradual, and the plan can change as pain settles and strength improves.

Putting treatment into practice

Use the prescribed exercises at the recommended frequency and keep the shoulder moving within a comfortable range. Reduce repeated overhead work, heavy lifting or reaching behind the back for a short period if these clearly provoke symptoms, then reintroduce them gradually. Support recovery with good sleep positioning and by sharing the actual demands of work, childcare or sport with the therapist. Do not judge progress only by pain during one exercise; easier dressing, reaching and sleep can be useful signs. If symptoms are not changing, the diagnosis and programme may need review.

When shoulder pain needs assessment

Seek urgent care for sudden inability to lift the arm after injury, a visibly deformed shoulder, severe swelling, new numbness or a cold or discoloured hand. Arrange medical review for fever, unexplained swelling, rapidly worsening pain or pain that is strong at rest and not settling. Persistent weakness, repeated catching or symptoms that fail to improve with a tailored programme may need further examination. Stop an exercise that causes sharp pain, new tingling or clear loss of strength and contact your clinician.
