How effective is physiotherapy for piriformis syndrome?

Short answer
Physiotherapy may reduce buttock pain and improve movement in piriformis syndrome by combining hip strengthening, mobility work, nerve-friendly loading and activity changes.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Assessment area
- Lower back, hip, pelvis, buttock and sciatic-nerve symptoms
- Treatment emphasis
- Comfortable movement plus progressive hip and trunk capacity
What this means
Piriformis syndrome describes irritation around the deep buttock muscles and nearby sciatic nerve. Pain can spread into the back of the thigh and may worsen with prolonged sitting, climbing or repeated hip movement.
A physiotherapist can examine the lower back, hip, pelvis and leg because similar symptoms can arise from more than one source. Treatment is therefore based on the examination rather than assuming that the piriformis muscle is the only cause.
What to do
Your programme may include gentle hip movement, strengthening of the gluteal muscles, trunk control and gradual exposure to sitting, walking or sport. Stretching is not automatically helpful; a stretch that increases leg tingling should be stopped and discussed.
Break up long sitting periods, vary positions and increase activity in small steps. Notice whether symptoms travel farther down the leg, become more intense or linger after exercise, since that information helps guide the next progression.
When to seek care
Seek an assessment if buttock or leg pain persists, interferes with sleep or walking, or is not improving with activity adjustments. Get urgent medical help for new bladder or bowel difficulty, numbness around the saddle area, or progressive leg weakness.
Questions for your doctor
Ask how the hip, lower back and nerve have been assessed, which movements are safe, whether leg tingling changes the plan, and how to build back sitting or exercise tolerance.

Questions people ask
Is stretching the piriformis enough?
Stretching alone may not address the reason symptoms persist and can aggravate nerve-related pain in some people. A broader programme is often more useful.
How long does physiotherapy take to help?
The response varies with the source of pain, nerve sensitivity, activity demands and consistency. Your clinician can adjust the plan when symptoms are not trending in the right direction.
