How effective is perineal repair for postpartum haemorrhage?

After birth, bleeding may come from a vaginal or perineal tear, the placental site in the uterus, retained tissue, or a problem with the uterus tightening. Stitches close a visible tear and may reduce bleeding from that injury. They cannot stop bleeding from inside the uterus or correct a clotting problem. The maternity team therefore assesses the source while treating the haemorrhage, sometimes using medicines, procedures or surgery as well as repair.

Tell the team how much you are bleeding and mention dizziness, weakness, breathlessness or worsening pain. A clinician may examine the birth canal, check the uterus and monitor pulse and blood pressure. If a tear is found, it can be repaired with anaesthesia and appropriate lighting. Follow discharge instructions about wound care, pads and review, and arrange help at home while strength returns.

Call emergency services for rapidly increasing bleeding, fainting, confusion, severe breathlessness, chest pain or cold clammy skin. Contact your maternity service urgently if bleeding becomes heavier again, clots are large or repeated, fever develops, or the repair becomes increasingly painful or swollen. Postpartum haemorrhage can worsen quickly, so do not wait for a routine appointment.

Ask where the bleeding came from, what was done to control it, whether the uterus was also treated, and what warning signs matter after discharge. Ask when your blood count, wound and emotional recovery should be reviewed.
