How effective is palliative care for malignant ascites?

Malignant ascites is a build-up of fluid in the abdomen related to cancer. It can cause swelling, tightness, pain, early fullness, nausea, reduced movement or breathlessness. Palliative care considers the cause, how quickly fluid is returning, your general health and what you want to be able to do. A clinician may discuss medicines, treatment directed at the cancer, or drainage of fluid through a procedure. Drainage can reduce pressure for a time, but fluid may return and the team will balance relief with risks such as infection, bleeding, low blood pressure or fluid shifts. Nutrition, bowel care, sleep and emotional support are also part of the plan.

Note changes in abdominal size, pain, eating, bowel movements, urination, weight, movement and breathing. Contact your team when fullness makes it hard to eat, sleep or move, rather than waiting until discomfort is extreme. Ask how to care for a drainage site if a procedure is planned, and do not change salt, fluid or diuretic use without medical advice. Loose clothing, smaller meals and supported positioning may improve comfort for some people.

Seek urgent help for severe or rapidly increasing abdominal pain, fever, fainting, confusion, repeated vomiting, inability to pass urine or sudden breathlessness. After drainage, report redness, warmth, leaking fluid, worsening pain or feeling faint. A tense abdomen with new breathing difficulty needs prompt assessment because the cause may require immediate treatment.
