How effective is osteotomy for Perthes disease?

Perthes disease affects the blood supply and development of the femoral head. As the hip changes shape, the treatment goal may be to keep the femoral head well positioned inside the socket while it heals and remodels. An osteotomy changes the angle or position of a bone around the hip to improve that containment.

The operation cannot restore damaged bone instantly, and the final hip shape develops over time. Doctors consider the child’s age, hip motion, X-ray findings, stage of the condition, and whether the femoral head can be contained. A child with a stiff or severely deformed hip may need a different approach or treatment before surgery is considered.

The care team may use repeated examinations and imaging to track hip position and growth. Before surgery, ask how the planned correction supports containment and what movement will be protected. Afterward, the child may need crutches, a brace, or limits on weight-bearing for a period set by the surgeon. Physiotherapy can help maintain safe hip movement and rebuild strength without overloading the healing bone.

Keep activity within the agreed limits even when pain improves. Follow-up imaging matters because the hip’s shape changes during healing, and the plan may be adjusted as the child grows.

Contact the treating team promptly for increasing hip or thigh pain, fever, wound drainage, a new leg-position change, or a sudden loss of movement. Seek urgent help for severe swelling, a cold or pale foot, new weakness, or breathing difficulty after surgery. During Perthes disease, worsening limp, persistent night pain, or reduced hip movement deserves review rather than waiting for the next routine visit.
