How effective is operative hysteroscopy for uterine fibroids?

A hysteroscope enters through the vagina and cervix, allowing the surgeon to see the inside of the uterus. This route is most relevant for submucosal fibroids that bulge into the cavity. Removing a cavity-distorting fibroid may help heavy bleeding or improve the space available for a pregnancy.

Fibroids located mainly within the uterine muscle or on the outside surface usually need a different approach. Even when a hysteroscopic procedure is suitable, the result depends on the fibroid’s size, attachment, depth, and whether it can be removed safely in one session.

Review the ultrasound or other imaging with your gynaecologist and ask exactly where the fibroid sits. Discuss whether the goal is bleeding control, fertility planning, or both, and whether staged treatment or another operation is more appropriate.

Follow preparation and recovery instructions carefully. Keep a record of bleeding and pain, and attend follow-up if your clinician wants to confirm that the cavity has healed or that no significant tissue remains.

Contact your surgical team urgently for heavy bleeding, severe pain, fever, fainting, shortness of breath, or foul-smelling discharge after the procedure. These signs require assessment rather than watchful waiting.

Arrange review if bleeding remains heavy, periods change markedly, or symptoms return. Seek pregnancy care promptly for bleeding or significant pelvic pain if you conceive after treatment.

Ask whether the fibroid is submucosal, how deeply it enters the uterine wall, whether one procedure is likely to be enough, what alternatives exist, and when pregnancy or other treatment can safely resume.
