How effective is chemotherapy before surgery for rectal cancer?

Preoperative treatment can reduce the tumour before surgery and may make the operation more manageable. The team considers scans, examination findings, bowel symptoms and the amount of disease around the rectum. Treatment may be combined with radiotherapy or other medicines when that fits the cancer’s location and features. Imaging can show change, but the tissue removed at surgery gives additional information about how much cancer remains.

Discuss the treatment sequence, planned scans and the intended operation with the colorectal oncology team. Tell them about bowel changes, weight loss, medicines and any previous reactions to treatment. During chemotherapy, follow guidance on hydration, food and infection precautions. Keep contact details for the treatment unit available, and ask in advance how a missed dose or a change in bowel function should be handled.

Call the treatment team promptly for fever, repeated vomiting, inability to drink, new severe abdominal pain, worsening rectal bleeding or a blocked bowel pattern. Seek emergency help for fainting, severe weakness, sudden breathing difficulty or heavy bleeding. These symptoms can need assessment whether or not they began after a treatment session.
