How effective is laminectomy for cauda equina syndrome?

A laminectomy removes part of the back of a vertebra to create space for compressed nerves. In cauda equina syndrome, pressure may affect the nerves controlling the legs, bladder, bowel and sensation around the saddle area. The operation treats compression; it cannot instantly restore nerves that have already been injured.

Some symptoms improve quickly, while weakness, altered sensation or bladder and bowel problems may recover more slowly or remain. The underlying cause, the level of compression, the severity of findings and the time before decompression all shape the outlook. Rehabilitation may include walking practice, strength work, bladder or bowel support and help with daily activities.

New difficulty passing urine, loss of bladder control, bowel control changes, numbness around the inner thighs or genitals, or worsening weakness in both legs needs emergency assessment. Do not wait for a routine appointment or try to manage these signs with home exercises. Tell clinicians when the symptoms began, whether they are progressing and whether back or leg pain changed.

After surgery, follow wound, lifting and movement instructions and attend neurological and rehabilitation reviews. Report changes in bladder emptying, bowel function, leg strength or sensation so support can be adjusted.

Cauda equina symptoms are an emergency. Seek immediate medical help for new urinary retention or incontinence, bowel incontinence, saddle numbness, rapidly worsening leg weakness or severe pain with neurological change. After laminectomy, urgent review is also needed for fever with wound problems, sudden weakness, loss of bladder control or severe worsening pain.
