How effective is hydrotherapy for frozen shoulder?

Short answer
Hydrotherapy may reduce discomfort and make gentle shoulder movement easier in frozen shoulder, but it cannot instantly restore motion or replace a personalised treatment plan.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Potential benefit
- Supported, gentler shoulder movement with possible comfort during exercise.
- Expectation
- Improvement is gradual and varies with the stage of frozen shoulder.
What happens in the pool
Warm water can relax surrounding muscles and buoyancy can support the arm during controlled movement. A therapist may use the pool for pendulum movements, assisted elevation, rotation within comfort, and posture work. This can provide a gentler setting for practice when land exercises feel difficult, but progress is usually gradual and should respect the irritated shoulder.
Frozen shoulder, also called adhesive capsulitis, involves pain and restriction around the shoulder capsule. The stage of the condition and your available range help determine which exercises are appropriate. Read more in [Frozen Shoulder](/conditions/frozen-shoulder/).
Making sessions useful
Have the shoulder assessed before starting so the programme matches your movement limits. Practise the prescribed range without forcing a sharp or catching pain, and use the therapist’s instructions about frequency and recovery. Note whether dressing, reaching, sleeping, or washing becomes easier; these everyday tasks are useful measures of progress.
Tell the therapist about diabetes, recent surgery, skin problems, numbness, or balance difficulties. Hydrotherapy can complement pain management and home exercises. The treatment approach is outlined in [Hydrotherapy](/treatments/physiotherapy-and-rehabilitation/hydrotherapy/).
When to seek care
Contact your clinician if pain is steadily worsening, the shoulder becomes hot and swollen, or you develop fever or drainage after a procedure. Seek urgent assessment for chest pain, severe breathlessness, a new weak or numb arm, or sudden loss of movement after an injury.
Questions for your appointment
Ask which shoulder movements are safe now, whether warmth or pool exercise suits your stage, how to measure progress, what pain level means you should stop, and when the diagnosis or treatment plan should be reviewed.

Questions people ask
Can hydrotherapy thaw a frozen shoulder immediately?
No. It may make movement more comfortable, but recovery and range usually require a planned programme over time.
Can I force the shoulder farther in warm water?
No. Move within the advised range; forcing can aggravate pain and make the programme harder to sustain.
